Immunization and Insurance

Immunization forms and Child Enrollment forms are required for children to attend the preschool. The immunization form will be provided to you at Fall Orientation and the Child Enrollment will be available for completion on-line. Both these forms must be complete and returned to the school before your child may attend class.
There is also a group insurance fee of $10 for each child. This non-deductible policy covers the child to, from and during school. Information is available at the Fall Orientation Meeting in September and the fee is paid at that time.
