Enrollment Forms

If your student is officially enrolled for fall, please print and complete the Enrollment Form and bring to the parent information night on Thursday, Sept. 4. Parents of 2 year olds must complete both forms.

If you are unable to print the forms, please plan to arrive 15 minutes early on Thursday to complete them before the meeting begins. Thank you!
